This is a heavily interactive web application, and JavaScript is required. Simple HTML interfaces are possible, but that is not what this is.

It’s a bit of a workaround, but if you have a Wordpress website the free plugin “Contact Form 7” can be connected to MailChimp and has the ability to send a receipt email.
