This is a heavily interactive web application, and JavaScript is required. Simple HTML interfaces are possible, but that is not what this is.

It’s the macOS equivalent of the dialog box that appears on iOS asking a user to enable notifications. On macOS, they’re opt-out; on iOS, they’re opt-in. But macOS notifications are so terribly designed that you need to hover over them to see the action buttons.
