As for the argument that lower wage workers are somehow inherently less desirable — the ludicrous that the entire hospital can be run be surgeons — I am similarly baffled as to how anyone finds it intuitive. I wrote on this in the case of Australia here https://doi.org/10.1111/1467-8462.12522
