This is a heavily interactive web application, and JavaScript is required. Simple HTML interfaces are possible, but that is not what this is.

Actually, we as employers can (and do) ask the insurance funds whether non-consecutive absences are part of the same illness or not. The answer is usually no, but sometimes yes.
